Informed Consent and Release Form

Members must complete this prior to booking ceremony.

I, the undersigned, acknowledge and agree to the following terms of participation in the spiritual and religious ceremonies of Centro Espiritualista Luz Do Vegetal Church Inc.

1. Purpose and Spiritual Intent

I understand that all Church ceremonies, teachings, and sacraments — including the use of the Sacrament of Ayahuasca — are conducted exclusively for religious and spiritual purposes. These practices are not medical, psychiatric, or psychotherapeutic treatments.

I affirm that I am over the age of 18, legally competent, and participating voluntarily.

2. Health and Personal Responsibility

I affirm that I have truthfully disclosed all relevant medical and psychological conditions, medications, and treatments to the Church prior to participation. I understand that withholding or falsifying such information may endanger my well-being and the safety of others.

I have also read the Ceremony Preparation Commitment in full and agree to follow the recommended guidelines on self-care both before and after ceremony. 

If I have any physical or psychological concerns, I have consulted a licensed medical or mental health professional prior to participating and have also discussed with a Church leader.

3. Nature of Spiritual Guidance

I understand that any teachings, reflections, or guidance provided by Church leaders are spiritual in nature and may include symbolic or moral interpretations. These should not be considered medical, psychological, or therapeutic advice.

I accept full responsibility for how I interpret my spiritual experiences.

4. Conduct & Respect

I have read and agree to the Code of Conduct.I agree to participate in all Church ceremonies and activities with reverence, integrity, and mindfulness toward others. CELDV is dedicated to fostering a sacred and inclusive environment for all. The Church maintains a zero-tolerance policy for any form of sexual, emotional, or physical harassment or violence, as well as discrimination of any kind — including race, gender, sexual orientation, nationality, religion, or any other protected status.

Any incident of inappropriate behavior or violation of personal boundaries must be reported immediately to Church leadership. All reports will be treated with discretion, compassion, and seriousness.

5. Privacy and Confidentiality

I will maintain confidentiality regarding the identities and personal experiences of all participants. I also understand that recording, photography, or solicitation of any kind is not permitted during ceremonies or on Church premises without written authorization.

6. Release of Liability and Indemnification

In consideration of being permitted to participate in Church ceremonies, I hereby release, waive, and discharge CELDV, its leaders, volunteers, affiliates, and agents from any and all claims, demands, or causes of action arising from my participation.

I further agree to indemnify and hold harmless the Church and its representatives against any liabilities, costs, or expenses resulting from my actions.

This release extends to my heirs, executors, administrators, beneficiaries, and assigns. Each provision of this agreement is independent and remains enforceable even if another provision is found invalid.

7. Legal and Ethical Acknowledgment

I understand that CELDV operates as a religious institution under protections afforded by law concerning freedom of religion and expression.

I acknowledge that participation in Church ceremonies is a spiritual act of faith — not a commercial or therapeutic transaction — and that the Church does not guarantee or promise any specific experiences or outcomes.

8. Voluntary Participation and Informed Choice

I confirm that my participation is entirely voluntary, and that I may cancel my membership in the Church at any time.  I have had the opportunity to ask questions about this form, and all have been answered to my satisfaction.

I understand that any donations made to the Church are spiritual offerings and are therefore non-refundable.

9. Acceptance 

I have read and fully understood this Informed Consent and Release Form. I agree to honor the Church’s Code of Conduct, Rules and Regulations, and Preparation Commitment. 

My signature below signifies that I knowingly, voluntarily, and intelligently consent to participate in the Church’s religious and spiritual ceremonies, and I accept all associated terms.
